    In fact yes.

    Every player has a ragdoll animation. You can add new stuff to the model tied to a new bone, and move it so it's visible only in the ragdoll animation.

    You'd have to recompile all other animations though, because of the changes made in the model (new bones and edited mesh).

    What do you want changed when its a ragdoll?

    I'm not sure if TF2 specifically supports this, but you might be able to use material proxies which take entity data and output a material variable from it. https://developer.valvesoftware.com/wiki/List_Of_Material_Proxies

    The Health and PlayerHealth could be what you're looking for. I know for a fact it works with Garry's Mod, but you'll have to experiment within TF2.
